Standards of care in perfusion: should not continuous in-line blood gas monitoring be one?
نویسنده
چکیده
What is a standard of care? Who defines standard of care? And when does new technology, such as continuous in-line blood gas monitoring (CILBGM), become a standard of care? Perhaps the most productive and practical way to view standards of care in perfusion is as, "the best perfusion management we can provide for our patients within reason and affordability." Today, CILBGM offers a tangible patient care benefit to perfusion management-real-time, continuous monitoring of the patient's physiologic responses to cardiopulmonary bypass, allowing the perfusionist to adjust bypass accordingly. At less than $200 per case, CILBGM is not excessively priced. Certain basic standards of practice for perfusion, along with the commitment to actively investigate new techniques and equipment, could help assure quality in patient care. If, as in the case of CILBGM, new technology contributes materially to the safety and well-being of a patient, we would be remiss not to endorse it as a standard of quality care.

The effects of continuous blood gas monitoring during cardiopulmonary bypass: a prospective, randomized study--Part I.
The use of continuous in-line blood gas management (CILBGM) is steeped in controversy concerning its potential utility and impact on patient outcomes. The purpose of this study was to determine whether the use of CILBGM results in improved quality of patient care. Fifty-nine patients were enrolled in a Institutional Review Board-approved, prospective, randomized study. An in-line blood gas moni...

Introduction: The recovery ward is a vital unit to care patients awaking from anesthesia and is a standard requirement for the operating room. Recovery ward is located adjacent to the operation room and is easily accessible to trained and skilled individuals. The unit must have adequate equipment for surveillance and monitoring of patients and required medication should also be provided. Meth...
متن کاملImproving cardiopulmonary bypass: does continuous blood gas monitoring have a role to play?
The CDI 500 (Terumo Cardiovascular Systems, Ann Arbor, MI) is an in-line blood gas monitoring device that has been used in clinical practice for over a decade. Few randomized studies have evaluated the value of this device with respect to improved perfusion management. We routinely use automated continuous quality indicator programs to assess perfusion management. The aim of this study is to in...
متن کاملA System for Continuous Estimating and Monitoring Cardiac Output via Arterial Waveform Analysis
Background: Cardiac output (CO) is the total volume of blood pumped by the heart per minute and is a function of heart rate and stroke volume. CO is one of the most important parameters for monitoring cardiac function, estimating global oxygen delivery and understanding the causes of high blood pressure. Hence, measuring CO has always been a matter of interest to researchers and clinicians. Sev...
متن کاملComparison of direct and indirect blood pressure in patients under esophagectomy surgery: a prospective observational study
Background: Arterial pressure is one of the most important physiological variables and often needs to be monitored repeatedly or continuously in perioperative period. Arterial pressure monitoring is one of the standard monitoring in operating room. During general anesthesia, blood pressure can be measured by using a noninvasive arterial pressure method or continuous invasive arterial pressure b...
